Thank you for serving with us. We want to make sure that you have a seat during our gatherings.

If you're planning to serve and attend both services, please check both 9am and 11am.  If you will only attend the 9am, please check only that box.  Also, we'd ask that your household register separately for the service they are planning to attend, we will make sure to seat you together.

You will also receive a reminder message on Sunday morning to go over the health checklist.
